GLP-1 Weight Loss Medications

Semaglutide and Tirzepatide are injectable medications that help with chronic weight management in adult patients. They are administered by injection into the area immediately under the skin using a small needle. Semaglutide and Tirzepatide belong to a class of medications called glucagon-like peptide-1 (GLP-1) agonists, which mimic the hormone GLP-1 in your body. This hormone slows down how fast your stomach empties food (called gastric emptying), stimulates your pancreas to release insulin (lowering blood sugar levels), and it blocks a hormone that causes your liver to release sugar (glucagon).

Together, these functions can help you feel less hungry, causing you to eat less food and ultimately lose weight. Like other prescription weight-loss drugs, Semaglutide and Tirzepatide are intended to be used as part of an overall weight-loss plan. It is indicated for people who are obese or overweight, and who have failed to lose enough weight with diet and exercise alone — not for people who want to lose just a few pounds. We only prescribe these medications to individuals with a Body Mass Index over 25.

The goal when using Semaglutide or Tirzepatide is to take a dose that suppresses your appetite without causing side effects that are not tolerable. Based on your body mass index we prescribe a starting dose that increases over the first month. The dosage is increased gradually over time until the patient reaches a suitable maintenance dose. We have found this method starts effective weight loss soon after starting therapy while still being very well tolerated for most patients. Healthy weight loss is defined as 1-2 lbs of weight per week. Research has shown that weight loss at a faster rate may cause negative side effects including muscle loss, metabolic changes, nutritional deficiencies, gallstone formation, dehydration, electrolyte imbalance, fatigue and hair loss.

Semaglutide/B12

Semaglutide is a medication that belongs to a class of drugs known as glucagon-like peptide-1 (GLP-1) receptor agonists. Originally approved for the treatment of type 2 diabetes, Semaglutide has also shown potential for weight management. In fact, Semaglutide has been approved by the U.S. Food and Drug Administration (FDA) for chronic weight management in adults with obesity or overweight who have at least one weight-related comorbidity.

Here are some key points regarding Semaglutide for weight loss:

  1.  Mechanism of action: Semaglutide works by mimicking the action of a naturally occurring hormone called GLP-1. GLP-1 helps regulate blood sugar levels, suppresses appetite, and promotes a feeling of fullness after meals. By activating GLP-1 receptors in the brain, Semaglutide can help reduce food intake and increase satiety.
  2.  Effectiveness: Clinical trials have shown that treatment with Semaglutide can lead to significant weight loss. In a large-scale study called the STEP trial, participants receiving Semaglutide along with lifestyle interventions achieved greater weight loss compared to those receiving a placebo. On average, individuals using Semaglutide lost around 15% of their initial body weight over a 68-week period.
  3.  Administration: Semaglutide for weight management is typically administered as a once-weekly subcutaneous injection. The medication comes in a pre-filled pen, and the injection is usually self-administered.
  4.  Safety considerations: Like any medication, Semaglutide has potential side effects. Common side effects include nausea, diarrhea, vomiting, and constipation. These side effects tend to diminish over time. In rare cases, more serious side effects such as pancreatitis have been reported. It is important to discuss potential risks and benefits with a healthcare professional before starting Semaglutide treatment.
  5.  Lifestyle modifications: Semaglutide is typically prescribed as part of a comprehensive weight management program that includes dietary changes, increased physical activity, and behavioral counseling. These lifestyle modifications are essential for long-term success and should be maintained even after discontinuing Semaglutide.
  6. B12 is a water-soluble vitamin normally found in foods of animal origin like fish, meat, poultry, eggs, milk, and other dairy products. It helps keep your body’s blood and nerve cells healthy and helps make DNA. Low levels cause fatigue, headache, nausea, GI upset, weakness as cognitive problems. Studies show that Vitamin B12 aids in the treatment tolerance with GLP-1 agonists (like Semaglutide) by reducing the most common side effects of nausea and gastrointestinal intolerance.

Tirzepatide/Glycine/B12

While very similar to Semaglutide as a GLP-1 receptor agonist, Tirzepatide also acts on gastric inhibitory polypeptide (GIP) as an agonist.

Gastric inhibitory polypeptide (GIP), also known as glucose-dependent insulinotropic peptide, is a hormone naturally released by the cells in the small intestine in response to food intake, particularly the ingestion of carbohydrates and fats. It plays a role in regulating glucose metabolism and stimulating insulin release from the pancreatic beta cells.

Here are some key points about gastric inhibitory polypeptide (GIP):

  1. Glucose regulation: GIP is involved in glucose homeostasis, meaning it helps maintain stable blood sugar levels. When you eat a meal containing carbohydrates or fats, GIP is released into the bloodstream. It stimulates the release of insulin from the pancreas, which helps lower blood glucose levels by promoting the uptake and storage of glucose in cells.
  2.  Insulin secretion: GIP has an incretin effect, meaning it enhances insulin secretion in response to elevated blood sugar levels. It acts by binding to specific receptors on the pancreatic beta cells, promoting the release of insulin. This mechanism helps ensure that the body responds appropriately to the rise in blood glucose after a meal.
  3.  Lipid metabolism: In addition to its effects on glucose metabolism, GIP also has some influence on lipid metabolism. It promotes the synthesis and storage of lipids (fats) in adipose tissue, which contributes to the regulation of overall energy balance.
  4.  Interaction with GLP-1: GIP and another hormone called glucagon-like peptide-1 (GLP-1) are part of a class of hormones known as incretins. Both GIP and GLP-1 are released from the intestine in response to food intake and stimulate insulin secretion. They work together to regulate glucose metabolism and maintain glucose homeostasis.
  5.  Involvement in satiety: While GIP is primarily known for its role in insulin secretion and glucose regulation, it may also have an impact on appetite regulation. Some research suggests that GIP may play a role in promoting feelings of satiety (fullness) and reducing food intake.
Studies show that Vitamin B12 aids in the treatment tolerance with GLP-1 agonists (like Tirzepatide) by reducing the most common side effects of nausea and gastrointestinal intolerance
  7. Glycine is a nonessential Amino Acid that is both produced by the body and in protein rich food sources.  It is the main structural protein of connective tissue and has anti-inflammatory properties.  Studies also show that Glycine improves sleep and neurological function.
